# Systemic BPC-157 and Human Tissue Healing: research update

Dated 5 October 2026. Local prepublication addendum; archived grade unchanged.

Animal structural-repair findings and cellular pathway signals remain distinct from controlled human healing. The retrieved human pilots address local symptoms or short-term intravenous tolerability, rather than the exact systemic structural-healing claim.

## Research timeline

Electronic year is used where recorded; issue year is retained in each source card. The chronology is selected, not exhaustive, and does not establish priority or causation.

| Year | Primary study | Design and finding | Inference limit |
|---|---|---|---|
| 2003 | [Gastric pentadecapeptide BPC 157 accelerates healing of transected rat Achilles tendon and in vitro stimulates tendocytes growth.](https://pubmed.ncbi.nlm.nih.gov/14554208/) | Rat Achilles transection plus cell culture. BPC-157 improved reported functional, biomechanical and histological tendon outcomes. | Rodent injury and culture findings do not establish human systemic efficacy. |
| 2004 | [Protective effects of pentadecapeptide BPC 157 on gastric ulcer in rats.](https://pubmed.ncbi.nlm.nih.gov/15052688/) | Rat gastric-ulcer experiments. Intramuscular and intragastric dosing reduced injury and supported repair in the tested models. | Rat mucosal repair is not a controlled human gastrointestinal outcome. |
| 2006 | [Achilles detachment in rat and stable gastric pentadecapeptide BPC 157: Promoted tendon-to-bone healing and opposed corticosteroid aggravation.](https://pubmed.ncbi.nlm.nih.gov/16583442/) | Rat tendon-to-bone repair. Treatment improved healing measures and opposed corticosteroid aggravation. | Acute surgical injury differs from chronic human tendinopathy. |
| 2010 | [Pentadecapeptide BPC 157 (PL 14736) improves ligament healing in the rat.](https://pubmed.ncbi.nlm.nih.gov/20225319/) | Rat ligament transection. Treatment improved functional, biomechanical and histological measures. | Species, route and injury-model boundaries remain. |
| 2010 | [The promoting effect of pentadecapeptide BPC 157 on tendon healing involves tendon outgrowth, cell survival, and cell migration.](https://pubmed.ncbi.nlm.nih.gov/21030672/) | Explant and tendon-cell assays. Outgrowth, migration and stress survival accompanied FAK/paxillin phosphorylation. | Pathway association is not a human healing-mediation test. |
| 2017 | [Stable gastric pentadecapeptide BPC 157 in the treatment of colitis and ischemia and reperfusion in rats: New insights.](https://pubmed.ncbi.nlm.nih.gov/29358856/) | Rat colitis/ischemia experiments. Treatment improved vascular and mucosal findings in selected injury models. | Mechanistic intervention is model-specific; human structural healing was not tested. |
| 2021 | [Intra-Articular Injection of BPC 157 for Multiple Types of Knee Pain.](https://pubmed.ncbi.nlm.nih.gov/34324435/) | Uncontrolled knee-pain report. A local injection report described symptom benefit and called for imaging follow-up. | No concurrent control or verified faster structural healing; local administration is outside systemic scope. |
| 2022 | [Pharmacokinetics, distribution, metabolism, and excretion of body-protective compound 157, a potential drug for treating various wounds, in rats and dogs.](https://pubmed.ncbi.nlm.nih.gov/36588717/) | Rat and dog pharmacokinetics. Intact peptide cleared rapidly; intramuscular bioavailability differed by species. | Radiolabel distribution includes metabolites and does not establish human active-peptide exposure. |
| 2024 | [Effect of BPC-157 on Symptoms in Patients with Interstitial Cystitis: A Pilot Study.](https://pubmed.ncbi.nlm.nih.gov/39325560/) | Uncontrolled interstitial-cystitis pilot. Participants reported symptom improvement following bladder-area injections. | Bladder symptoms, local delivery and no concurrent control do not test the prespecified tissues. |
| 2025 | [Safety of Intravenous Infusion of BPC157 in Humans: A Pilot Study.](https://pubmed.ncbi.nlm.nih.gov/40131143/) | Two-person intravenous tolerability pilot. No adverse effects were reported during the short observation period. | Previously exposed adults and tiny sample cannot establish general safety or healing efficacy. |
| 2026 | [Effects of BPC-157 and TB-500 on Achilles tendon healing in rats: A histopathological and biomechanical study.](https://pubmed.ncbi.nlm.nih.gov/42542926/) | Exploratory rat tendon-repair comparison. Histological effects were reported; combination treatment lacked additional benefit. | Endpoint-specific results should not be recast as universal biomechanical superiority. |

## Research gaps

### G1: Does systemic BPC-157 accelerate human tendon or ligament structural healing?

Evidence anchors: [P14554208](https://pubmed.ncbi.nlm.nih.gov/14554208/), [P20225319](https://pubmed.ncbi.nlm.nih.gov/20225319/), [P34324435](https://pubmed.ncbi.nlm.nih.gov/34324435/), [P42542926](https://pubmed.ncbi.nlm.nih.gov/42542926/).

Competing explanation: Symptoms may improve through analgesia, rehabilitation or natural recovery without faster structural repair.

Discriminating test and controls: Registered randomized trial of a characterized product in objectively confirmed injury; identical rehabilitation, blinded imaging and mechanical/function endpoints.

### G2: Does it heal human gastrointestinal mucosal injury?

Evidence anchors: [P15052688](https://pubmed.ncbi.nlm.nih.gov/15052688/), [P29358856](https://pubmed.ncbi.nlm.nih.gov/29358856/), [P39325560](https://pubmed.ncbi.nlm.nih.gov/39325560/).

Competing explanation: Animal cytoprotection or local bladder symptom relief may not translate to gastrointestinal re-epithelialization.

Discriminating test and controls: Tissue-specific controlled trial with endoscopic/histological healing, systemic exposure confirmation and standard care held constant.

### G3: What exposure reaches the injured human tissue?

Evidence anchors: [P36588717](https://pubmed.ncbi.nlm.nih.gov/36588717/), [P40131143](https://pubmed.ncbi.nlm.nih.gov/40131143/).

Competing explanation: Product heterogeneity or metabolite signal may be mistaken for sustained active-peptide exposure.

Discriminating test and controls: Assay intact peptide and metabolites separately; establish batch identity, human PK, tissue-relevant PD and a prespecified exposure–response relation.

### G4: Are pathway signals necessary for repair and are effects independently reproducible?

Evidence anchors: [P21030672](https://pubmed.ncbi.nlm.nih.gov/21030672/), [P14554208](https://pubmed.ncbi.nlm.nih.gov/14554208/), [P42542926](https://pubmed.ncbi.nlm.nih.gov/42542926/).

Competing explanation: Phosphorylation may be downstream repair activity; positive-study concentration may reflect reporting or laboratory effects.

Discriminating test and controls: Independent preregistered injury-model replication, dose controls and pathway blockade/rescue with structural endpoints; report nulls and all planned outcomes.

## Methods and access

Case-specific Consensus records assisted gap discovery, chronology and targeted paper questions. Shared template wording about an amyloid-specific Deep follow-up is not evidence of a Deep run for this topic; the dated source records establish the actual use and access limits. Primary identifiers, selected abstract passages, correction notices and available primary-body passages were cross-checked. Generated absence cells, duplicate milestones and unsupported access claims were rejected.

This focused addendum does not establish 50–70 complete primary Methods/Results examinations per review. Baseline reading receipts and scores retain their original scope.

Checked timeline sources: 11; selected full-text checks: 1; abstract-excerpt checks: 10; metadata-only historical anchors: 0. Complete papers examined in this update: 0.

Historical scorecards declare schema 5.4. This enrichment uses HSEF v5.3 causal distinctions without retrospectively relabelling or rescoring those files.

Targeted paper-check scope: Rat and dog Results and material-identification Methods: rapid intact-peptide elimination, species-specific IM bioavailability and separately synthesized tritiated material. Human exposure was not measured.
