## Research gaps

Separate measured fetal exposure from birthweight as a mixed genetic and environmental proxy.

Triangulate non-genetic mortality/fertility selection; a null BMI polygenic-index comparison excludes only a measured genetic-selection explanation.

Do not infer nutrition-specific mediation from birthweight MR or cohort hazard ratios.

Identify selective developmental mediators with loss/rescue, including maternal food-intake and stress controls for steroid models.

Adult clinical follow-up must distinguish persistent susceptibility from accelerated deterioration and from childhood biomarkers.

Postnatal rescue and sex/window differences need prespecified replication rather than responder-defined narrowing.

## Boundary experiment

Follow a prospectively defined prenatal exposure intervention or credible policy natural experiment into adult incident disease, with fetal/maternal genotype and sibling/paternal controls, prespecified sex/window contrasts, quantified postnatal nutrition and selection. Pair it with cross-species prenatal mediator loss and selective rescue while matching maternal intake, stress and lactation. Retain all enrolled exposure groups and telemetry/function outcomes rather than selecting responders or counting epigenetic marks as clinical proof.
