Research gaps

Associative and limbic striatum are distinct anatomical hypotheses.

Historical challenge registration, expectancy controls and selective mediator evidence need fuller source recovery.

Treatment resistance and symptomatic remission complicate uniform excess.

Muscarinic efficacy does not demonstrate dopamine dispensability; EMERGENT-3 excluded TRS.

Prospective mechanistic stratification needs independent validation rather than retrospective response-group discrimination.

Study that would resolve the boundary

Prospectively register a selective human dopamine/D2 mediator comparison in anatomically and clinically predefined psychosis strata, measure target engagement and acute-effect expectancy/blinding, use blinded symptom trajectories and an orthogonal nondopaminergic comparator, and prospectively test responses without selecting eligibility from observed treatment outcome.
