# Adversarial review - epigenetic-field-cancerization

Level before -> after: Γ Moderate -> Γ Moderate (unchanged).
Findings: 2 Major, 1 Minor. Validator: PASS (0 fail, 1 warn, 31 pass); the single warning is the standing-reject v5.claim_form 'phenomenon' notice.

## ADVERSARIAL REVIEW AND APPROVAL

**Authoring model:** Sol 6.1. **Adversarial reviewer:** Claude Opus 5.5 (Anthropic). **Date:** 2026-10-14. This is the revised (9 October 2026) edition; the original edition was not adversarially reviewed, so this is the first adversarial review of this case.

**Scope of checks:** All 63 reference PMIDs re-fetched with the PubMed MCP tool get_article_metadata (title, first author, journal, year, DOI): 63/63 resolve to the stated record; no retraction or expression of concern. 12 load-bearing claims spot-checked against PubMed abstracts: refs 11, 21, 42, 44 (Yu, PMID 25061879), 49 (Yang, PMID 37143122), 53, 60 (Niwa, PMID 20124475), 63 (Yamada, PMID 40240063), plus the MLH1 constitutional-epimutation and normal-tissue clonal-sequencing claims. Scoring logic checked against the archived HSEF v5.3 framework, with particular attention to the declared scope-matched aggregation rule. The four Consensus artifacts named in the provenance block were confirmed to exist on disk. Known upstream defects checked: collective-author dropping (none present), empty DOI fields (none), erratum "PMIDnot supplied" strings (none), rendering defects (one found, corrected).

**Findings and corrections**

| ID | Severity | Location | Original -> corrected | Evidence |
|---|---|---|---|---|
| EF-1 | Major | Consensus provenance reconciliation block (also consensus-addendum.md) | "The earlier adversarial approval refers to its original edition; ..." -> "...The original edition of this review carried no adversarial approval; the adversarial review of record is the one recorded in ADVERSARIAL REVIEW AND APPROVAL below." | The original edition, hsef_v53_popularity_ranks_46_50_2026-10-09/epigenetic-field-cancerization/epigenetic-field-cancerization.review.md, contains no adversarial banner and no ADVERSARIAL REVIEW AND APPROVAL section, and this slug is absent from the 2026-10-06 approved set. The 9 October revision asserted an approval that does not exist. This is a regression introduced by the revision, not an inherited error. |
| EF-2 | Minor | Β/Γ/Δ boundary paragraph | "step1 ... step2 ... step3 ... step4 ... step5" -> "step 1 ... step 5" | Known missing-space rendering defect. |
| EF-3 | Major | scorecard final_score.confidence_rationale | "...8 selected partial-body examinations; density, heterogeneous controls and incomplete access set the confidence..." -> rationale resting on density and reproducibility: the two engineered p16 reports share one allele and are not independent lines, the organoid result is culture- and transplantation-dependent, field evidence is largely cross-sectional, prospective cohorts carry wide intervals, and organ/segment nulls and one abstract-versus-body discrepancy remain unresolved | Lead policy: reading access is not evidence density or reproducibility and cannot set a band. Re-derived under the two-step test, the band stays **Moderate** - the review documents substantive reproducibility and evidence-quality reservations, well short of the High text's "extensive independent replication across labs, models, and assay modalities". No level or confidence change. |

**Rejected findings (checked, no change)**

- Scope-matched aggregation: applied correctly. SC5 is the only subclaim whose scope matches the registered claim (spontaneous endogenous epimutations independently initiating and sustaining a clonal field) and it sets the Γ headline; the Ε subclaims SC2 and SC3 are scope-restricted to an engineered locus and to defined inflammatory settings and are reported separately, exactly as the rule requires. The published sensitivities (weakest-link Β, primary-subclaim Γ, scope-matched Γ) are arithmetically consistent with the matrix.
- "Best-supported rival": correctly rendered here. models_evaluated[0] is M2, is_nominal false, rank_in_tournament 1; the review's own nominal model (M1, "Spontaneous autonomous epigenetic cancer field") is correctly placed at rank 4 and is not presented as the rival.
- Yu (PMID 25061879): "Engineered p16(Ink4a) promoter hypermethylation ... increased the incidence of spontaneous cancers" matches the abstract, as does the absence of a required engineered oncogene and the accelerated onset in the germline-mutation/somatic-epimutation compound. The review's caution that the two p16 reports share one allele is correct and is applied to the independence basis.
- Yamada (PMID 40240063): 1624 followed participants, 27 incident cancers, age- and sex-adjusted HR 5.7 with a wide interval - all match the abstract, and the review does not promote the predictive result to causal mediation.
- Asada (PMID 25379950): three preselected markers, annual endoscopy, exclusion of first-year events as likely occult baseline cancer - all match the abstract; the quoted adjusted quartile HR is the multivariate figure from the body, and the univariate 2.17 (1.07-4.41) is consistent with it.
- Niwa (PMID 20124475): inflammation suppression with cyclosporin A did not affect colonization but blocked methylation induction, and post-eradication levels fell yet stayed above uninfected controls - the review's "partly persistent component" wording matches.
- Momentum "Mechanistic only": the named artifacts are completed mechanistic and prospective reports, and the review explicitly declines to claim an ongoing native spontaneous-field independence experiment. Correct under the lead's momentum policy.
- REVIEWER NOTES and the Consensus block are mutually consistent: the notes record discovery and correction prompts with one abstract-summary/original-body aspect comparison and zero provider-returned verbatim body passages. No contradiction of the kind seen elsewhere in this batch.
- Reference 42 prints "BrafV600E-Induced Tumorigenesis" and reference 63 prints "eradication of H. pylori" where the PubMed title field drops the superscript and the italicised species name: rejected per the stated convention.
- Print/e-pub year differences of +/-1 at refs 2, 11, 15, 16, 29, 53 and 62: acceptable under the stated convention.
- Validator warning `v5.claim_form` ("entry_form is 'phenomenon'"): rejected per the standing convention. The restatement grades a causal claim - spontaneous hypermethylation independently initiating and sustaining field expansion **and** subsequent carcinogenesis - not a bare topic.

**Residual limitations**

- 55 of 63 sources were verified at abstract/status depth. Per lead policy this bounds what was checked and is not a determinant of the confidence band.
- The linked MSH2 erratum body and the left-colon abstract-versus-body single-locus discrepancy remain unexamined; the review already flags both in KNOWLEDGE GAPS and does not quote the disputed estimate.
- scorecard provenance_header.generated_by is unchanged; authorship is recorded here as Sol 6.1 per the mandate.
- Package-level files were not edited; propagation strings are listed in the agent report.

**Approval:** Following adversarial review and the corrections listed above, the revised document is approved. — Claude Opus 5.5 (Anthropic), 2026-10-14.
