Research update · 5 October 2026

Research gaps and timeline

Human associations, animal transfer experiments and clinical microbiome interventions address different causal steps. Recent adjunctive FMT findings strengthen an intervention line in depression, while human mediator specificity and generalization across psychiatric disorders remain unresolved.

This dated addendum adds selected evidence and tests that could resolve the remaining questions. It does not regrade the archived review.

Questions the next study should resolve

Each question identifies a limit in the checked evidence. A missing search result alone does not establish a gap in the entire field.

Which microbial function carries a human depression-treatment effect?

Competing explanation: Taxa can be markers of diet, medication, inflammation or a broader microbial functional state.

Discriminating test: Replicated, strain/function-resolved randomized interventions with repeated metabolites, symptoms and prospective mediator analysis; stabilize medication and diet.

Evidence: PMID 36473852 · PMID 42309058 · PMID 27491067

Is the vagus a necessary human route?

Competing explanation: Immune or circulating metabolite routes may contribute alongside vagal signals.

Discriminating test: Human autonomic and mediator measurements within controlled interventions, complemented by model blockade/rescue; do not equate autonomic correlation with necessity.

Evidence: PMID 37131071 · PMID 28483500 · PMID 42309058

When do positive and null clinical effects occur?

Competing explanation: Formulation, diagnosis, baseline microbiota, concurrent treatment or gastrointestinal improvement may modify effects.

Discriminating test: Independent multisite replication with prespecified diagnosis/strain strata, matching expectancy and gastrointestinal symptoms, and a clear primary psychiatric outcome.

Evidence: PMID 28483500 · PMID 28068788 · PMID 39504580 · PMID 42309058

Does benefit persist after dosing and engraftment changes?

Competing explanation: Short-lived symptom improvement may reflect concurrent antidepressant response or nonspecific treatment effects.

Discriminating test: Blinded longer follow-up with repeated microbial function and clinical relapse measures; record adherence, additional care and donor/product identity.

Evidence: PMID 42309058 · PMID 28068788 · PMID 36473852

Selected research timeline · 8 reports

Electronic publication dates are used where recorded, with issue years shown separately. This is a selected chronology, not a claim of scientific priority or independent replication for every entry.

  1. 2016

Gut microbiome remodeling induces depressive-like behaviors through a pathway mediated by the host's metabolism.

    Human-donor transfer to germ-free mice · Online 2016-04-12; issue 2016 · PMID 27067014

Depression-donor microbiota produced depressive-like behaviour and metabolic changes.

What it cannot establish: Germ-free host conditions and cross-species transfer bound applicability.

  2. 2016

Transferring the blues: Depression-associated gut microbiota induces neurobehavioural changes in the rat.

    Human-donor transfer to microbiota-depleted rats · Online 2016-07-25; issue 2016 · PMID 27491067

Depression-associated donor microbiota induced behavioural and physiological changes in recipients.

What it cannot establish: Rat phenotypes and donor confounding do not prove human disease sufficiency.

  3. 2017

A double-blind, randomized, placebo-controlled trial of Lactobacillus helveticus and Bifidobacterium longum for the symptoms of depression.

    Probiotic randomized low-mood trial · Online 2017-01-10; issue 2017 · PMID 28068788

The tested formulation did not provide evidence of a symptom benefit.

What it cannot establish: A null formulation/population result does not exclude every microbiome intervention.

  4. 2017

Probiotic Bifidobacterium longum NCC3001 Reduces Depression Scores and Alters Brain Activity: A Pilot Study in Patients With Irritable Bowel Syndrome.

    Probiotic placebo-controlled IBS pilot · Online 2017-05-05; issue 2017 · PMID 28483500

Depression scores and brain-activation measures improved with the tested strain.

What it cannot establish: IBS population and strain-specific intervention do not establish a general psychiatric mechanism.

  5. 2022

Gut microbiome-wide association study of depressive symptoms.

    Two-cohort microbiome association · Online 2022-12-06; issue 2022 · PMID 36473852

Several taxa were associated with depressive symptoms across cohorts.

What it cannot establish: Association and inferred neuroactive potential do not establish effector delivery to the brain.

  6. 2023

Gut microbiota changes require vagus nerve integrity to promote depressive-like behaviors in mice.

    Mouse transfer plus vagotomy · Online 2023-05-02; issue 2023 · PMID 37131071

Vagotomy prevented the tested microbiota-induced behavioural and neurogenic effects.

What it cannot establish: Mouse route necessity is not established human vagal mediation.

  7. 2025

Probiotic Formulation for Patients With Bipolar or Schizophrenia Spectrum Disorder: A Double-Blind, Randomized Placebo-Controlled Trial.

    Bipolar/schizophrenia probiotic randomized trial · Issue 2025 · PMID 39504580

Gut-health outcomes improved; a broad psychiatric benefit was not established.

What it cannot establish: Gastrointestinal or exploratory memory effects are not disorder-wide symptom efficacy.

  8. 2026

Adjunctive fecal microbiota transplantation for major depressive disorder: A randomized, double-blind, placebo-controlled trial.

Adjunctive depression FMT randomized trial · Online 2026-06-17; issue 2026 · PMID 42309058

The report linked improved adjunctive outcomes with engraftment, bile acids and mediation analysis.

What it cannot establish: Statistical mediation does not directly randomize the proposed bile-acid mechanism.

Sources, reading depth and Methods

Case-specific Consensus records assisted gap discovery, chronology and targeted paper questions. Shared template wording about an amyloid-specific Deep follow-up is not evidence of a Deep run for this topic; the dated source records establish the actual use and access limits. Primary identifiers, selected abstract passages, correction notices and available primary-body passages were cross-checked. Generated absence cells, duplicate milestones and unsupported access claims were rejected.

Results Par32: vagotomy or sham surgery preceded transfer by two weeks; vagotomy prevented the tested behavioural/neurogenesis deficits in mice.

This focused addendum does not establish 50–70 complete primary Methods/Results examinations per review. Baseline reading receipts and scores retain their original scope.

Selected full-text checks: 1; abstract checks: 7; metadata-only anchors: 0. Complete-paper examinations credited in this update: 0.

New enrichment: HSEF v5.3 causal distinctions. Archived scorecard schema: 5.4; retained as recorded.
