### Consensus consistency validation — 9 October 2026

**Current grade:** Γ
**Grade changed:** no

**Scope:** Retrospective, transparently unblinded consistency check of load-bearing primary records already in the topic inventory. This is not a new peer review and does not establish historical Consensus use.

**Consensus queries:**
- `gut microbiota causal systemic disease human randomized fecal microbiota transplantation autoimmune arthritis type 2 diabetes cardiovascular disease mechanisms primary studies`
- `primary human randomized fecal microbiota transplantation trial Kootte 2017 Vrieze 2012 Yu FMT-TRIM autoimmune arthritis segmented filamentous bacteria Wu 2010`

**Saved raw Consensus records:**

**Original-primary comparisons:**
- **PMID 20620945** (claims C1) (PubMed abstract_metadata in baseline inventory): Segmented filamentous bacteria drive Th17-associated autoimmune arthritis in a mouse model; bounded animal causality, not broad human systemic primacy.
- **PMID 24009397** (claims C2) (PubMed abstract, reopened 2026-10-09): Fecal microbiota from obesity-discordant twins transferred body/fat-mass and metabolic phenotypes to germ-free mice; diet-dependent and animal transfer.
- **PMID 32150549** (claims C9) (PubMed abstract_metadata in baseline inventory): FMT-TRIM placebo-controlled pilot in obesity did not improve the primary metabolic endpoint; weighs against assuming human systemic metabolic effects from transfer.
- **PMID 28978426** (claims C8) (PubMed abstract_metadata in baseline inventory): Lean-donor FMT effect on insulin sensitivity depended on baseline microbiota and was short-term; not disease initiation or general systemic cause.

**Discovery/reconciliation versus source incorporation:** Initial Consensus query surfaced mostly reviews. Targeted query returned an atopic-dermatitis RCT (irrelevant), a FMT-TRIM meeting abstract, and reviews; fetch verifies FMT-TRIM concerns an obesity pilot, not proof of cross-system human causation. The field Delphi consensus mentioned by the page is not service use. Original primary lines support selected animal causal effects and mixed/limited human treatment evidence.

**Conclusion:** No grade change: Γ for microbial composition as a broad primary cause across distant human organ diseases. Historical mention is field-level Delphi consensus, not Consensus service provenance; fresh searches are retrospective only.

This dated supplement adds source reconciliation to the original review. Its original narrative, registered scope, source inventory and grades remain historical records. No earlier adversarial approval has been verified for this edition at the supplement date. This provenance supplement has no separate approval and is not an independent or blinded adjudication. New sources described here are supplemental and are not counted in the original inventory.
