Research gaps

Selective diet-specific clinical mediation remains weaker than particle causality.

Cis-MUFA and cis-PUFA, individual SFA species and food matrices need separate effect estimates.

Historical trans-fat exposure and incomplete data recovery remain unresolved; contamination must not be asserted as an established explanation of nulls.

Mean response and responder-defined follow-up are distinct populations.

More diverse clinical populations and complete correction/supplement checks are needed.

Study that would resolve the boundary

Independently run, preregistered long-term isoenergetic replacement trial with fixed SFA species, cis-PUFA/cis-MUFA comparators, matched food matrices, trans-fat measurement, sustained apoB separation and blinded adjudicated ASCVD outcomes; prospectively test mediation and competing metabolic pathways.
