  "title": "What this review means",
  "includeSubclaimIllustrations": false,
  "redteamReviewed": true,
  "accessNotice": "Source checking used bounded original primary abstract passages/status records and selected original partial-body passages. No complete-abstract or complete-paper audit, or independent scientific peer review, is claimed. Detailed access counts and limitations appear in the review.",
  "overview": "Galleri detects real cancers, but a cancer-mortality benefit remains unestablished; the 2026 primary stage endpoint was null and a lower stage IV rate in a secondary endpoint that could not be formally tested requires clinical follow-up.",
  "implications": "The preregistered primary-subclaim rule returns Γ from SC2. Fifty-eight original reports were examined, covering Galleri/other MCED diagnostic work, prospective implementation, real mortality-based single-cancer screening comparators, and separate MRI/DTC pathways. No positive Galleri mortality crossing is counted. The current primary stage endpoint failed and a favorable secondary stage IV signal remains; neither proves a mortality benefit or a mortality null. Weakest-link returns Α from the broad companion claim, while primary-subclaim and scope-matched return Γ. The review preserves genuine detections and legitimate established screening benefits without assigning them to a different assay. The framework's 'epiphenomenon' label means a causal role in reducing cancer deaths is not established by the evidence, not that screening has no effect; no effect on cancer deaths has yet been shown in either direction.",
  "limitations": "58 primary reports with 3 selected partial-body examinations, plus 2 records added in the 2026-11-02 adversarial correction. Confidence reflects evidence density and reproducibility, not access depth or correctness: the ceiling is well characterised because the randomized Galleri trial, a large prospective study and a real-world series all leave no Galleri mortality contrast. The grade applies to the exact registered claim; stronger findings for narrower subclaims remain separate. No complete-paper audit or blinded calibration is claimed. This educational review does not establish an individual treatment recommendation."
