# Serotonin deficiency hypothesis of MDD: research update

Dated 5 October 2026. Local prepublication addendum; archived grade unchanged.

Serotonergic perturbations can reveal relapse vulnerability and altered release capacity, but a simple baseline-deficiency account of all major depression requires evidence beyond antidepressant efficacy or candidate-gene association. State, subtype and measurement specificity remain central.

## Research timeline

Electronic year is used where recorded; issue year is retained in each source card. The chronology is selected, not exhaustive, and does not establish priority or causation.

| Year | Primary study | Design and finding | Inference limit |
|---|---|---|---|
| 1990 | [Serotonin function and the mechanism of antidepressant action. Reversal of antidepressant-induced remission by rapid depletion of plasma tryptophan.](https://pubmed.ncbi.nlm.nih.gov/2184795/) | Depletion in antidepressant-remitted patients. Tryptophan depletion transiently reversed remission in a subset. | Drug-maintained vulnerability does not establish low serotonin as the initial disease cause. |
| 1994 | [Serotonin and the neurobiology of depression. Effects of tryptophan depletion in drug-free depressed patients.](https://pubmed.ncbi.nlm.nih.gov/7944875/) | Depletion in untreated depressed patients. Rapid depletion did not immediately worsen depression; later responses varied. | The relation is not a simple linear deficiency-to-symptom function. |
| 2003 | [Influence of life stress on depression: moderation by a polymorphism in the 5-HTT gene.](https://pubmed.ncbi.nlm.nih.gov/12869766/) | Longitudinal stress-by-genotype cohort. A serotonin-transporter genotype interaction with stress was reported. | A candidate-gene interaction is not a direct serotonin measurement and later large studies challenged the finding. |
| 2004 | [Neural and behavioral responses to tryptophan depletion in unmedicated patients with remitted major depressive disorder and controls.](https://pubmed.ncbi.nlm.nih.gov/15289275/) | Medication-free remitted MDD randomized crossover. Depletion induced symptoms and altered regional metabolism in remitted patients versus controls. | Susceptibility after past disease is not prospective proof of an initiating deficiency. |
| 2009 | [Effect of acute antidepressant administration on negative affective bias in depressed patients.](https://pubmed.ncbi.nlm.nih.gov/19755572/) | Acute reboxetine randomized experiment. Emotional-processing bias changed before subjective mood improved. | Noradrenergic treatment demonstrates a competing treatment route, not direct serotonin restoration. |
| 2019 | [No Support for Historical Candidate Gene or Candidate Gene-by-Interaction Hypotheses for Major Depression Across Multiple Large Samples.](https://pubmed.ncbi.nlm.nih.gov/30845820/) | Large-sample candidate-gene analyses. Historical candidate-gene and interaction findings were not supported. | Null candidate variants do not directly measure or exclude every serotonergic mechanism. |
| 2022 | [Brain Serotonin Release Is Reduced in Patients With Depression: A [11C]Cimbi-36 Positron Emission Tomography Study With a d-Amphetamine Challenge.](https://pubmed.ncbi.nlm.nih.gov/36635177/) | Serotonin-release challenge PET. Release capacity was lower in the studied depressive-episode group. | Small cross-sectional challenge study cannot establish temporal precedence or universal deficiency. |

## Research gaps

### G1: Does a measurable deficit precede a first untreated episode?

Evidence anchors: [P36635177](https://pubmed.ncbi.nlm.nih.gov/36635177/), [P30845820](https://pubmed.ncbi.nlm.nih.gov/30845820/), [P7944875](https://pubmed.ncbi.nlm.nih.gov/7944875/).

Competing explanation: Release differences may be state effects or downstream consequences of illness.

Discriminating test and controls: Longitudinal never-medicated at-risk and incident-case cohorts with repeated validated release measurements, matched controls and treatment histories.

### G2: Which biologic subtype is vulnerable to depletion?

Evidence anchors: [P2184795](https://pubmed.ncbi.nlm.nih.gov/2184795/), [P15289275](https://pubmed.ncbi.nlm.nih.gov/15289275/), [P7944875](https://pubmed.ncbi.nlm.nih.gov/7944875/).

Competing explanation: Prior treatment and recurrent illness may create vulnerability distinct from initial causation.

Discriminating test and controls: Prespecified comparisons of untreated acute, remitted medication-free and treated groups; sham crossover, depletion confirmation and symptom monitoring.

### G3: How specific is the release measurement?

Evidence anchors: [P36635177](https://pubmed.ncbi.nlm.nih.gov/36635177/).

Competing explanation: Challenge pharmacology, tracer binding, receptor regulation or nonserotonergic effects may influence the signal.

Discriminating test and controls: Independent replication with test–retest assessment and orthogonal measures; prespecify challenge/tracer controls and distinguish release from receptor density.

### G4: Does treatment work through serotonin normalization or another route?

Evidence anchors: [P2184795](https://pubmed.ncbi.nlm.nih.gov/2184795/), [P19755572](https://pubmed.ncbi.nlm.nih.gov/19755572/), [P36635177](https://pubmed.ncbi.nlm.nih.gov/36635177/).

Competing explanation: Emotional processing and downstream network adaptation may improve symptoms without correcting a baseline deficit.

Discriminating test and controls: Repeated mediators and clinical endpoints within randomized treatment comparisons; preserve timing and test competing noradrenergic/psychological pathways.

## Methods and access

Case-specific Consensus records assisted gap discovery, chronology and targeted paper questions. Shared template wording about an amyloid-specific Deep follow-up is not evidence of a Deep run for this topic; the dated source records establish the actual use and access limits. Primary identifiers, selected abstract passages, correction notices and available primary-body passages were cross-checked. Generated absence cells, duplicate milestones and unsupported access claims were rejected.

This focused addendum does not establish 50–70 complete primary Methods/Results examinations per review. Baseline reading receipts and scores retain their original scope.

Checked timeline sources: 7; selected full-text checks: 0; abstract-excerpt checks: 7; metadata-only historical anchors: 0. Complete papers examined in this update: 0.

Historical scorecards declare schema 5.4. This enrichment uses HSEF v5.3 causal distinctions without retrospectively relabelling or rescoring those files.

Targeted paper-check scope: Primary abstract/metadata checked. Additional Consensus full-text details remain provider-reported because a separately readable body was unavailable in this update.

Editorial correction/limit: This focused timeline contains seven distinct primary studies rather than padding the count with meta-analyses. An unrelated PMID returned during identifier verification was excluded.
