Research timeline and gaps: Menopause Hormones: Symptoms, Prevention and Testosterone

Selected publication milestones, not an exhaustive history or discovery priority claim.

1998 — PEPI compared systemic regimens and showed vasomotor relief without a matching generalized cognitive/affective benefit. Original PMID9840563.

2002 — A randomized laboratory experiment tested the predicted sweating-threshold response using physiological symptom recording. Original PMID11872200.

2002 — WHI combined-regimen prevention outcomes forced separation of symptom relief from chronic disease benefit and harm. Original PMID12117397.

2003 — WHIMS tested incident dementia in an older enrolled population rather than assuming symptom efficacy conferred brain protection. Original PMID12771112.

2008 — APHRODITE established an indication-specific testosterone counterexample: modest HSDD benefit without concomitant estrogen. Original PMID18987368.

2014 — MsFLASH directly compared estradiol, venlafaxine and placebo for baseline bothersome vasomotor symptoms. Original PMID24861828.

2014 — A selective verbal-learning testosterone signal remained bounded by null cognitive domains and general wellbeing. Original PMID24716847.

2016 — ELITE showed why early-start surrogate vascular changes and hard disease prevention must be distinguished. Original PMID27028912.

2018 — REPLENISH evaluated a defined oral estradiol/progesterone capsule with symptom and finite endometrial endpoints. Original PMID29889748.

2024 — A tested fezolinetant regimen failed to outperform placebo in MOONLIGHTI, mapping a nonhormonal context limit. Original PMID38818888.

2025 — OASIS3 expanded contemporary neurokinin-pathway symptom evidence while separating confirmatory and descriptive follow-up endpoints. Original PMID40920404.

Research gaps

The complete endocrine-to-thermoregulatory mediator sequence is not isolated by ordinary efficacy trials.

Allocation-guess assessment and objective symptom sensing are incompletely integrated in large hormone trials.

Long-term hard endpoints for contemporary transdermal estradiol/micronized progesterone begun near menopause need direct assessment rather than extrapolation from oral CEE/MPA or observational continuation.

Physiological-dose testosterone requires replicated patient-important nonspecific cognition, fatigue and muscle-function endpoints; supraphysiologic dose signals do not establish ordinary replacement benefit.

Rare breast, vascular and liver harms require formulation-specific long-term surveillance and cannot be excluded by small short trials.

Boundary experiment

Run a prospectively registered, adequately powered symptomatic-menopause trial of a measured systemic estradiol regimen, placebo and a mechanistically distinct nonhormonal comparator, with identical contact, allocation-guess checks, objective thermal/skin-conductance sensing and daily patient-important symptoms. Embed baseline-defined age/stage and route strata and a blinded randomized-withdrawal phase; test the hypothesized mediator without conditioning on response. A separate long-horizon contemporary-regimen prevention trial is required for SC3.
