## Findings

Full table (AR-1 to AR-4), the nine rejected findings and the residual limitations are in the `## ADVERSARIAL REVIEW AND APPROVAL` section of `vitamin-d-deficiency.review.md`. The two revision-introduced regressions:

- **AR-1 (Major).** The known systematic artifact, present here. The 9 October integration spliced in a "Consensus consistency validation" section naming a Consensus Pro thread by URL and listing preserved provider artifacts, while leaving the preserved REVIEWER NOTES sentence stating that no Consensus work was performed. Repaired by scoping the original sentence to the original 2026-10-09 review pass and recording that the dated validation is a later, separately scoped supplement that changed no grade or source total. Neither block was deleted.
- **AR-2 (Major).** The validation block's closing paragraph asserts that "the earlier approval does not certify this supplement", presupposing an earlier adversarial approval. Verified against `hsef_v53_remaining_priority_01_05_2026-10-09/vitamin-d-deficiency/vitamin-d-deficiency.review.md`, which has no approval section or banner. Rescoped in both the review and `consensus-addendum.md`.

Remaining: **AR-3** confidence rationale rewritten off access depth, and **AR-4** Divergence artifact added.

## Note on the confidence band (lead two-step test)

This was the closest call in my group. Striking access depth leaves two very large independent randomized trials on different continents plus a genetically informed literature, which looks like the High text. The band nevertheless stands at Moderate, because access was not the only thing holding it down: the review itself makes the decisive point that the registered exposure is *deficiency* while the large trials supplemented broad populations with mostly modest baseline deficiency, so they are indirect for the registered estimand. The one trial recruited in a genuinely deficient population tests a single disease in children; the secondary and tertiary reports share parent-trial participants; achieved-concentration analyses condition on a postrandomization variable; subgroup and genotype signals are unreplicated; and the Mendelian-randomization evidence carries instrument and nonlinear-stratification assumptions. Those are evidence-quality determinants, not access.

## Dated-fact check

The most dated-fact-sensitive item is handled correctly and needed no correction: the originally significant VITAL autoimmune estimate (HR 0.78, 95% CI 0.61 to 0.99) is not left standing, and the review reports the updated randomized-period estimate of HR 0.85 (0.70 to 1.04) after further case confirmation, while refusing to treat the two years of post-intervention observation as continued randomized exposure.

## Strings for the lead to propagate to package-level files

None. No level, confidence, role, momentum value, source count or claim count changed. The earlier edition at `hsef_v53_remaining_priority_01_05_2026-10-09/vitamin-d-deficiency/` is a divergent copy and was **not** overwritten.

**Approval:** Following adversarial review and the corrections listed above, the revised document is approved. — Claude Opus 5.5 (Anthropic), 2026-10-14.
